GP Cluster Profiles 2013



A set of profiles has been produced which provides information on a number of key indicators in relation to GP cluster populations. The profiles contain descriptive analysis of the registered population in terms of geographic spread, age/sex structure, deprivation, rural/urban characteristics and access to primary care. The profiles also contain information on recorded disease burden and age adjusted variation for a number of chronic conditions.

Key Messages

The Public Health Wales Observatory has produced a set of profiles to support the development of GP clusters. Health boards have formally developed arrangements for small groups of GP practices to work collaboratively to develop services in the community, serving populations of between 30 and 50 thousand patients.

The concept of ‘GP clusters’ was first set out in Setting the Direction the Welsh Government’s primary and community services strategic delivery programme. GPs in the clusters play a key role in supporting the ongoing work of a locality network (in some areas these are known as neighbourhood networks). GP clusters are charged with working together and with partners to meet local need.

Seven documents have been published, one for each health board containing a health board overview followed by a profile of each cluster within the health board area.

The profiles aim to provide information on a number of key indicators in relation to GP cluster populations. They contain descriptive analysis of the registered population in terms of its geographic spread, age/sex structure, deprivation, rural/urban characteristics and access to primary care. In addition, the profiles contain information on chronic disease for a number of conditions that have a high impact on services:

  • Chronic obstructive pulmonary disease
  • Asthma
  • Coronary heart disease
  • Heart failure
  • Hypertension
  • Epilepsy
  • Diabetes

Information on disease burden and age-adjusted variation is included.

The interactive data files produced to support the area profiles found above.  It is recommended that these spreadsheets are used in conjunction with the health board reports and the supporting technical guide (.pdf, 1.4MB). The profile documents are restricted to the clusters within the host health board, the data files allow all clusters in Wales to be seen. They provide an opportunity to see changes in patterns for a particular indicator and enable the copying of charts for use in documents and presentations.

  • Age/sex breakdown (.xls, 776KB): population pyramids are used to show the proportion of the registered population by sex in grouped age bands

  • Deprivation (.xls, 719KB): the proportion of the registered population residing in LSOAs by WIMD 2011 fifths of deprivation

  • Rurality (.xls, 729KB): the proportion of the registered population residing in LSOAs by ONS rural/urban definition

  • Drive Time analysis (.xls, 714KB): showing a model-based time taken for patients to reach their registered main practice

  • Burden of chronic disease (.xls, 987KB): analysis of actual and relative recorded chronic disease burden for:

    • Chronic obstructive pulmonary disease

    • Asthma

    • Coronary heart disease

    • Heart failure

    • Hypertension

    • Epilepsy

    • Diabetes

This is the first time that demographic and chronic disease burden indicators have been presented at the GP cluster level and it is anticipated that further products will be produced in the future following evaluation and feedback regarding this initial work.
