Please provide a list of the current Primary Care Clusters in Wales, with the name of the GP account (including NHS ID) that aligns with each PCC. Please provide this data as an Excel or CSV file.

Thank you for your recent request dated 21th October 2022. Public Health Wales hold the information on Clusters (x60 across Wales) and the names of the GP practices aligned to each cluster but we do not hold the GP NHS ID details.
