Please can I request the following information under the Freedom of Information Act 2000, with regards to fire safety door inspections at NHS Wales Trusts.
• How many fire safety doors are there in total across all trust buildings?
• What was the date of your most recent fire safety door inspection?
• How many/what percentage (whichever is easier) of your fire safety doors failed this inspection?
• How regularly do you carry out fire safety door inspections? i.e. annually, monthly, etc.
• If available, what is the main reason for any failures during your most recent fire safety door inspection?

Public Health Wales fire door survey programme is undertaken across 19 sites over a 12 month period, with the last of those sites tested on the 1st September. Over that 12 month period, we’ve seen a total of 227 failures from 295 doors, where reports have been received.
• How regularly do you carry out fire safety door inspections? i.e. annually, monthly, etc.
Our current policy/procedure which is due to be reviewed – will state that fire door surveys will be undertaken every 12 months as a minimum.
• If available, what is the main reason for any failures during your most recent fire safety door inspection?
The main reason for failures are issues with hinges/smoke seals/intumescent strips and gaps to frame, meeting edge or threshold.
It is important to note that Public Health Wales is the public health service for Wales and as such is an independent NHS Trust. This is NOT the same as NHS Wales. As an organisation we have no jurisdiction over the activities of Health Boards and frequently do not have access to the data that they produce.
