FOI 928 - Adequate Health Provision

Information Requested:

Under the provisions of the Freedom of Information Act 2000, please provide policy documentation which would describe all patient benefits accrued from being referred from Mid Wales to Hereford Hospital for treatment or examination at a facility currently under special measures, and additionally where no adequate public transport between Mid-wales and Hereford exists.

Information provided for the answer:

Thank you for your recent request dated 21st April 2022.
Public Health Wales do not hold this data, it is important to note that Public Health Wales is the public health service for Wales and as such is an independent NHS Trust. This is NOT the same as NHS Wales. As an organisation we have no jurisdiction over the activities of Health Boards and frequently do not have access to the data that they produce.
I believe that for Mid Wales, the Health Board you would need to contact would be Powys Teaching Health Board, but without the address details I cannot be certain so I have provided contact details for each Health Board below, I suggest you contact your local Health Board for the information you require.
