I am writing to submit a formal request for information under the Freedom of Information (FOI) Act 2000. My request relates to patient safety incident data required to be reported under the National Policy on Patient Safety Incident Reporting and Management. Please can you provide the following information:

For each of the above points, please provide data disaggregated by Health Board, Trust or Special Health Authority in NHS Wales from the date the policy was brought into effect to present.
Thank you for your recent request dated 6th September 2023. In relation to points 1 and 3 of the request, the below table summarises National Reportable Incidents (NRI) data for NRIs that have been coded by the reporting organisation as relating to ophthalmic services (variations for spelling have been included where identified) reported to the NHS Wales Delivery Unit (now part of the NHS Wales Executive) from 14 June 2021 to 20 September 2023. The level of harm the reporting organisation associated with the incident at the time the NRI report was made is also included.
In relation to point 2 of the request, the NHS Wales Executive does not collect data on patients who have categorised as health risk factor R1 as part of the NRI reporting processes so we are unable to respond to this point.
Please note the NHS Wales Executive are only able to provide data relating to Nationally Reported Incidents. Individual Health Boards, Trusts and Special Health Authorities in NHS Wales would need to be contacted directly in order to provide any detail in relation to locally reported incidents.
