Individual Sample Taker Reports

These reports show the number of samples taken by the sample taker each month, the number of avoidable repeats and a breakdown of the reasons for any repeat samples. A new report is generated every 3 months (February, May, August & November) and shows performance over a 12 month period.            
The latest report covers the period March 2020 to April 2021.  The time period is based on the date the NBS sample was received in the laboratory.

  • Enter the password provided by NBSW or your health board governance lead
  • Enter your sample taker ID code (NMC PIN or NBSW ID code) where indicated on the spreadsheet, and then click anywhere to display your data
  • Print or save your report.
       To Print: Go to File > Print                                           
To Save: Go to File > Save As > Browse to required folder > Use Save as type ‘PDF’. By saving to PDF this avoids you having to enter the password each time you open your saved version.
One bloodspot card can have multiple reasons for the repeat request. The number of reasons for repeat may therefore be greater than the total repeats.
A card with inadequate demographics, such as a missing date of sample, does not currently result in a repeat being requested. However, it does require laboratory time to obtain the missing information.
Sample takers must be registered on the NBSW Sample taker database and clearly record their name and sample taker ID code on the bloodspot cards in order to receive these reports. If you haven’t already registered, please complete the sample taker registration form.
