Primary and Community Care Development and Innovation Hub

The Primary and Community Care Development and Innovation Hub (the PC Hub) sits within the Primary Care Division.

The PC Hub was established in 2016 to ‘coordinate support for health boards and clusters, at a national level, in the delivery of the national plan for primary and community care in Wales (2015-2018) and facilitate coordinated delivery on a range of projects’.

The aspirations in that plan are continued via the National Strategic Programme for Primary Care, which contributes strategic direction to continuing support from the PC Hub.

The PC Hub annual work plan is agreed and overseen by a programme board with senior representatives from health boards, Public Health Wales, NHS Wales and the Welsh Government. The PC Hub Programme Board oversee two workstreams:

  • Supporting primary care transformation
  • Prevention in primary care settings

Annual reports documenting the work of the PC Hub can be found here on the Primary Care One Wales website.
