The below response is provided by the NHS Executive which is an NHS body hosted by Public Health Wales.
To respond to your request for information, it will be necessary to explain the current and previous national reporting requirements relating to the death of a patient known to Mental Health Services. Below is a timeline of the reporting requirements in keeping with your request for data back to 2014.
N.B. Please note that national reporting thresholds are different to organisational reporting requirements, meaning not all deaths reported at an organisational level will also be reported nationally.
2014 – 31 August 2020 - Organisations were required to submit a Serious Incident (SI) report to Welsh Government (WG) relating to any patient who died unexpectedly whilst in receipt of NHS funded healthcare. This is regardless of which service the patient was under and could therefore include a service user known to Mental Health Services.
In addition, and specific to Mental Health service users, organisations were required to submit an SI report where a patient known to Secondary Mental Health care services in the last 12 months completed or was suspected of completing suicide.
This information is not held by the NHS Executive and your enquiry will need to be directed to Welsh Government.
1 September 2020 – 13 June 2021 - Organisations were required to submit an SI report to the then NHS Wales Delivery Unit (now NHS Executive). Whilst classified as a serious incident, there was no requirement for there to be any adverse element of the patients care that was considered to have caused or contributed to the circumstances of their death at the time of reporting.
139 Incidents were reported in this period.
14 June 2021- 30 April 2024 N.B. National Reportable Incident (NRI) replaces the term Serious Incident (SI).
Organisations are required to nationally report the death of a Mental Health or Learning Disabilities service user where investigations determine there was an element of NHS funded healthcare which caused or contributed to the death. Organisations have up to 90 working days to conduct their investigations from the point they become aware of the death and report retrospectively. Alternatively, organisations can report the death earlier and later request to downgrade the incident if investigations determine there was no element of NHS funded healthcare that caused or contributed to the death.
161 Incidents were reported in this period.
