Information Requested:

May I make the following FOI request under the Freedom of Information Act 2000 (FOIA).  

  

Can you please email the following data for Total Orthodontics Wrexham (Unit K1 Ellice Way, Wrexham, Ll13 7YT and a trading name of Oasis Dental Care Limited) for the following 4 years between 2019-2022 (2019, 2020, 2021, 2022): 

  1. Total NHS Wales funding provided to Total Orthodontics Wrexham each year. 

  1. Total number of NHS patients actively being treated by Total Orthodontics Wrexham  each year. 

  1. Total number of NHS patients on the waiting list with Total Orthodontics Wrexham awaiting treatment each year. 

  1. How much funding was returned by Total  Orthodontics Wrexham to NHS Wales each year (e.g. as a result of failing to provide treatment). 

Please confirm data is from Jan-Dec or Apr-Mar each year.
