Tackling Diabetes Together Programme

Wales has the highest number of people living with diabetes in the UK. Today, more than 200,000 people in Wales are living with diabetes and by 2035 it is expected that there will be an additional 48,000 people living with the disease - equating to one in 11 adults. Nearly all of those living with diabetes in Wales have type 2 diabetes, a serious and sometimes lifelong condition which can cause major health problems.  

The Tackling Diabetes Together Programme is committed to bringing about a step-change in diabetes management and prevention across Wales. The programme is uniting the knowledge, research and expertise of NHS Wales including Health Boards, NHS Trusts, Public Health Wales, the Welsh Value in Health Centre, the clinical diabetes network and third-party partners, along with the lived experience of those with diabetes, to enable system-wide benefits of focusing on prevention and effective care for those with diabetes.   We aim to create a Wales where people live longer, healthier lives. By working together and working with people living with diabetes, we will drive system-wide improvements focused on prevention, early intervention, and enhanced care.  

By 2028 we aim to ensure that the programme will have achieved its two objectives - to have fewer people in Wales living with diabetes, as well as better care and outcomes for those already living with diabetes.
