Summary of changes to document:

In response to comments received as part of peer review, some feedback received from the local community and full quality assurance processes, the following is a summary of the main changes that have been made since the draft document shared with the Investigation Response Team meeting in public on 9 December 2010:

• Additional contextual information for readers including a background section (section 3 page 8);
• Some alteration of the terminology used and their description, in particular definitions for type I and II errors rather than false positive and false negative results;
• Added explanation of areas included in MSOA 020;
• Explanation added about years used (2001-8 for some and 2004-8 for others);
• Some additions to the discussion sections;
• Revision to the method of counting hospital admissions. These have resulted largely to changes in magnitude of rates described, but have had less impact on the pattern of health seen across the area and have not changed the statistically significance of any of the results previously described for the areas in the vicinity of Hanson cement;
• Revised definition for COPD cases;
• A summary of the indicators findings (Appendix A);
• More detail on the definitions of indicators used (Appendix B);
• A glossary (Appendix C).
