Note: Leaving feedback is different from making a complaint. To make a complaint, you must follow the guidance in the complaints section. Any complaints left via the feedback forms may not be able to be processed. 

We use what people tell us to understand the quality of service you have received from us.

Services such as any of our national Screening programmes, Help me Quit, The GP Exercise referral scheme, our Microbiology or Health Protection service or any of the community engagement activities we carry out.  Your feedback helps us to better under the experiences of people receiving the service and support us in quality improvements to ensure we strive for excellence.
