About this event

Communities4Change (C4C) Wales is an evidence informed, time limited approach which brings together people from multiple agencies with a common goal to enable and accelerate change to improve health and health equity.

Cwm Taf Morgannwg Healthy Housing partnership (CTMHHP), sponsored by Cwm Taf Morgannwg University Health Board, was selected by Public Health Wales (PHW) to test and evaluate the C4C Wales approach. Cwm Taf Morgannwg Healthy Housing Partnership is a multi-agency group which aims to improve the health and well-being of the people in Rhondda Cynon Taf, Merthyr Tydfil and Bridgend Local Authorities through improving the quality of housing and level of support to those who are homeless.

The proposed goal for the CTMHHP C4C collaborative was to develop a mechanism to share, interpret and use data for action across partners in relation to housing and health.

This webinar gave an overview of the C4C Wales approach, how the work of CTMHHP has evolved since participating in the C4C Wales pilot and key learning points from the evaluation.
