FOI 2023 202 - Cremation incident

 

Information Requested:

Link to news story - Wrong body cremated in shocking hospital mix up (msn.com) 

As you can see from the headlines in two or more papers you have mentioned that this is the hospitals fault  - The person that was wrongly cremated was Patrisha Randal.  

Please can you confirm this and also confirm this was an error on the hospitals side.
