Individual Sample Taker Reports

These reports show the number of samples taken by the sample taker each month, the number of avoidable repeats and a breakdown of the reasons for any repeat samples. A new report is generated every 3 months and shows performance over a 12 month period.

Sample takers must be registered on the NBSW Sample Taker Database and clearly record their Sample Taker ID Code on the bloodspot cards in order to receive these reports. If you have not already registered, please complete the sample taker registration form

• Enter the password provided by NBSW or your health board governance lead
• Click 'enable edit' at the top of the page
• Enter your sample taker ID code (NMC PIN, GMC number or NBSW ID code) where indicated on the spreadsheet, and then click anywhere to display your data
• Print or save your report.
